AbstractCadmiumisanenvironmentalpollutant,withrelevantexposuresatworkplacesandinthegeneralpopulation.Thecarcinogenicityhasbeenlongestablished,mostevidentfortumorsinthelungandkidney,butwithincreasingevidencealsoforothertumorlocations.WhiledirectinteractionswithDNAappeartobeofminorimportance,theinterfer-encewiththecellularresponsetoDNAdamage,thederegulationofcellgrowthaswellasresistancetoapoptosishavebeendemonstratedindiverseexper-imentalsystems.WithrespecttoDNArepairpro-cesses,cadmiumhasbeenshowntodisturbnucleotideexcisionrepair,baseexcisionrepairandmismatchrepair;consequencesareincreasedsuscep-tibilitytowardsotherDNAdamagingagentsandendogenousmutagens.Furthermore,cadmiuminducescellproliferation,inactivatesnegativegrowthstimuli,suchasthetumorsuppressorproteinp53,andprovokesresistancetowardsapoptosis.Particularlythecombinationofthesemultiplemechanismsmaygiverisetoahighdegreeofgenomicinstabilityincadmium-adaptedcells,relevantnotonlyfortumorinitiation,butalsoforlaterstepsintumordevelop-ment.Futureresearchneedstoclarifytherelevance
oftheseinteractionsforlowexposureconditionsinhumans.

Cadmiumisubiquitouslydistributedthroughouttheenvironment,attributabletonaturalsources,agricul-tureandmanifoldindustrialuses.Relevantexposuresoccuratworkplacesmainlyviainhalation,butalsointhegeneralpopulationpredominantlyviafood,tobaccosmokeandambientair.Eventhoughthetoxicityofcadmiumhasbeenknownfordecades,duringrecentyearstherehasbeenanongoingdiscussiononcadmium-inducedtoxicity,evenatcomparativelylowexposureconditions.Forexample,recentlytheEuropeanFoodSafetyAuthority(EFSA)hasloweredtheProvisionalTolerableWeeklyIntake(PTWI)of7lg/kgbwestablishedpreviouslybytheJointFAO/WHOExpertCommitteeonFoodAdditivestoaTWIof2.5lg/kgbwbasedoncadmium-inducednephrotoxicity(EFSA2009).Otherendpointsoftoxicologicalconcernarebonedemineralizationandcardiovasculardiseases(seecontributionsofTimNawrotandDavidBernhard,
